FAQs about Colorectal cancer

Question 1

Who is at higher risk of developing colorectal cancer?

Accepted Answer

People with certain risk factors have a higher chance of developing colorectal cancer. The strongest risk factor is increasing age. Although colorectal cancer is more common after age 50, the risk begins to rise from about age 45, and cases in younger adults have also been increasing.People with a family history of colorectal cancer or advanced colorectal polyps, especially in a first-degree relative, have an increased risk. Certain inherited conditions, such as genetic cancer syndromes, can also raise the risk.Lifestyle factors may also contribute. People who smoke, drink excessive alcohol, have obesity, follow a diet high in processed meats, or have low physical activity levels may have a greater risk.Certain medical conditions, including long-term inflammatory bowel diseases such as ulcerative colitis or Crohn’s disease, can increase colorectal cancer risk.People at increased risk may need to begin colorectal cancer screening earlier or undergo screening more frequently than average-risk individuals, based on their healthcare provider's recommendations.

Question 2

What is the survival rate of colorectal cancer?

Accepted Answer

The overall five-year relative survival rate for colorectal cancer is roughly 65%. This means about 65 out of 100 people diagnosed are alive at least five years later. However, this figure fluctuates significantly based on how early the cancer is detected and treated. The primary factor determining a patient's prognosis is the stage of the disease at the time of diagnosis. To determine life expectancy, physicians rely on the five-year relative survival rate provided by the American Cancer Society or the National Cancer Institute's SEER (Surveillance, Epidemiology, and End Results) database:Localized Stage: The cancer remains confined to the colon or rectum and has not spread to nearby lymph nodes or distant organs and hasn't spread outside, the five-year relative survival rate exceeds 90%.Regional Stage: If the cancer has spread to nearby organs or surrounding lymph nodes, the five-year survival rate drops to approximately 73% to 74%.Distant Stage: If the cancer has metastasized to distant organs (such as the liver or lungs), the five-year survival rate is between 13% and 15%.Early detection is paramount. When found early through regular screenings, colorectal cancer is highly curable. Conversely, a lack of colorectal cancer screening programs leads to many late-stage diagnoses, which heavily impacts mortality. It is important to remember that survival statistics are estimates based on large groups of past patients. A patient's individual outlook depends on many variables, including their age, overall health, specific type of treatment, and how their body responds to therapies. Recent advancements in targeted therapies and surgical techniques continue to improve long-term survival metrics. Question 3

Does colorectal cancer run in families?

Accepted Answer

Yes, colorectal cancer can run in families, with about one-third of all cases showing some familial component. However, only about 5% to 10% of colorectal cancers are directly hereditary, meaning they are caused by specific, inherited gene mutations passed down from a parent. The majority of colorectal cancer cases are "sporadic," occurring by chance in individuals with no known family history. These cancers result from acquired (somatic) gene mutations that accumulate over time due to aging, random DNA replication errors, and environmental or lifestyle factors. For cases that do run in families, the risk is typically classified into two main categories:Inherited or Hereditary SyndromesThese are rare genetic conditions directly caused by mutated genes.Accounting for roughly 1% to 4% of all cases, Lynch syndrome (hereditary non-polyposis colorectal cancer) stands as the most frequent hereditary form of the disease . Another notable condition is Familial Adenomatous Polyposis (FAP), which causes hundreds or thousands of polyps in the colon. Individuals who inherit these gene changes have a significantly higher lifetime risk of developing cancer, often at a younger age.Familial CancerIn many instances, multiple family members develop colorectal cancer, but no specific gene mutation can be identified. This aggregation is usually the result of a combination of shared genes, common environmental factors, and similar lifestyle habits within a household.Assessing Your RiskIf you have a first-degree relative—such as a parent, sibling, or child—diagnosed with colorectal cancer, having one first-degree relative with colorectal cancer generally increases your risk by about twofold, with an even higher risk if the relative was diagnosed before age 50 or if multiple first-degree relatives are affected. If that relative was diagnosed before age 50, your personal risk increases further. What You Can DoKnowing your family history is crucial. For individuals with a family history of colorectal cancer or advanced adenomas, clinical guidelines advise for some people with a first-degree relative diagnosed at a younger age or with multiple affected relatives, screening may begin at age 40 or 10 years before the youngest affected relative's age at diagnosis, whichever comes first. The exact recommendation depends on individual risk factors and current screening guidelines. Speaking with a doctor about genetic counseling and Genetic Testing for Cancer Risk can help determine if you carry a hereditary syndrome.

Question 4

I saw blood on the toilet paper or in the toilet bowl. Do I need a colonoscopy?

Accepted Answer

Blood in or on the stool, on the toilet paper, or in the toilet bowl is never normal, though it is often caused by benign conditions. Hemorrhoids and anal fissures are the most common causes of bright red blood, especially if the blood is only on the toilet paper or drips into the bowl. However, colorectal cancer can also cause bleeding, and the two conditions can coexist. The general rule is that any rectal bleeding in a person aged forty-five or older warrants a colonoscopy to rule out cancer. For younger individuals, a colonoscopy is still warranted if bleeding is accompanied by any other symptom such as a change in bowel habits, abdominal pain, unexplained weight loss, or a family history of colorectal cancer.A negative colonoscopy in a young person with bleeding and a normal examination may be reassuring, but the decision should be made by a gastroenterologist.Assuming that blood in the stool is from hemorrhoids without proper testing may lead to delayed diagnosis and treatment.

Question 5

Can colorectal cancer be cured if detected early?

Accepted Answer

Yes, colorectal cancer can often be cured when it is detected early, especially when the cancer is found before it spreads to other parts of the body. Early-stage colorectal cancer is usually easier to treat because the cancer is more likely to be completely removed with curative treatment, most often surgery. For many patients with early-stage colon cancer, surgery to remove the cancerous area can be the main treatment and may be curative. Depending on the stage and location of the cancer, chemotherapy, radiation therapy (particularly for rectal cancer), targeted therapy, or immunotherapy may also be recommended. The chances of successful treatment are generally higher when colorectal cancer is diagnosed at an early stage because the cancer has not yet invaded nearby tissues or spread to distant organs. Early detection through screening tests, such as colonoscopy and stool-based tests, can also help find precancerous polyps before they develop into cancer. However, the possibility of cure depends on several factors, including the cancer stage, tumor characteristics, overall health, and response to treatment. Advanced colorectal cancer that has spread to distant organs is usually not curable, although treatment can often control the disease for prolonged periods, relieve symptoms, and improve quality of life.Regular screening, recognizing warning signs, and getting medical advice early can greatly improve the chances of successful treatment and better outcomes.

Question 6

If a polyp is found and removed during my colonoscopy, am I cured?

Accepted Answer

Yes, if a polyp is completely removed during colonoscopy and the pathology report shows that the polyp was benign, meaning it contained no cancer cells, or showed only low-grade dysplasia, The polyp contained abnormal cells that were not cancer, and once it is fully removed, that specific polyp has been successfully treated. However, having a polyp is a marker of increased risk for developing future polyps. The patient will enter a surveillance program with repeat colonoscopy at an interval determined by the number, size, and histology of the polyps found. For patients with one or two tubular adenomas smaller than one centimeter, repeat screening is usually recommended after five to ten years. For three or more adenomas, or any adenoma larger than one centimeter, or any adenoma with villous features or high-grade dysplasia, the recommended interval is three years. If a polyp that was removed turns out to have a focus of early-stage cancer, called a T1 cancer, the patient may be cured if the cancer was entirely removed with clear margins and if there are no high-risk features such as lymphovascular invasion or poor differentiation. Such patients require closer surveillance.

Question 7

What is the difference between the symptoms of diverticulitis and colon cancer?

Accepted Answer

Diverticulitis and colon cancer can cause overlapping symptoms, but there are important differences. Diverticulitis typically causes sudden, severe pain in the left lower abdomen, often accompanied by fever, nausea, and a change in bowel habits, usually constipation. People often experience continuous and severe pain.Colon cancer more often causes vague, crampy abdominal pain that comes and goes, a persistent change in bowel habits lasting weeks to months, and bleeding that may be occult or visible.Fever is rare in colon cancer unless there is perforation or obstruction. A CT scan of the abdomen and pelvis can usually distinguish between the two conditions. However, after an episode of diverticulitis has resolved, a follow-up colonoscopy is recommended to exclude an underlying cancer that may have been masked by the inflammation.

Question 8

What is the difference between colon cancer and rectal cancer?

Accepted Answer

Colon cancer and rectal cancer are two types of colorectal cancer, but they develop in different areas of the large intestine. Colon cancer starts in the colon, which is the larger part of the intestine responsible for absorbing water and forming stool. Rectal cancer begins in the rectum, the final section of the digestive tract where stool is stored before passing out of the body.Although both cancers can cause similar symptoms, such as blood in stool, abdominal discomfort, and changes in bowel habits, their treatment approaches may differ.Although surgery is a common treatment for both cancers, rectal cancer is more likely than colon cancer to require radiation therapy, often combined with chemotherapy, because of its location within the pelvis. Both conditions are grouped together as colorectal cancer because they share many risk factors and develop from similar abnormal cell changes.

Question 9

Can colorectal cancer develop without symptoms?

Accepted Answer

Yes, colorectal cancer can develop without noticeable symptoms, especially in its early stages. Many people have no noticeable symptoms when colorectal cancer is small or at an early stage. This is why regular screening is important, even for people who feel healthy. Some people may notice changes in their normal bowel routine, including constipation, diarrhea, blood in stool, abdominal discomfort, or other symptoms such as tiredness and unintentional weight loss. Some symptoms can be mild or mistaken for common digestive problems, which may delay diagnosis. However, having these symptoms does not always mean someone has colorectal cancer, as many other conditions can cause similar problems.Screening tests, such as colonoscopy and stool-based tests, can help find colorectal cancer early or detect polyps before they become cancerous. Early detection often provides more treatment options and can improve outcomes.

Question 10

Can colon polyps turn into colorectal cancer?

Accepted Answer

Yes, some colon polyps can turn into colorectal cancer over time, but not all polyps become cancer. Colon polyps are abnormal growths that develop on the inner lining of the colon or rectum. Most colon polyps are benign (noncancerous), but some—particularly adenomatous polyps and certain serrated polyps—can develop into colorectal cancer if left untreated. The process of a precancerous polyp developing into colorectal cancer often takes about 10–15 years, although it can occur more quickly in some individuals. During this time, cells inside the polyp may develop genetic changes that allow them to grow uncontrollably and become colorectal cancer. However, removing risky polyps during screening procedures, such as a colonoscopy, can help prevent cancer from developing. Not every polyp has the same cancer risk. Some types, such as adenomatous and certain serrated polyps, are considered higher risk, while other noncancerous polyps usually do not become cancer. The risk can also depend on the polyp’s size, number, and features seen under a microscope.Regular colorectal cancer screening is important because it can detect and remove polyps before they have a chance to become cancer. Early detection and prevention through screening can reduce the risk of developing colorectal cancer.
